Who should use this application.
Any General Contractor that acts as a General Prime Contractor or Subcontractor that is involved in New Home Construction of either Single Family Custom Homes, Multiplex units, Apartments or Tract Subdivisions. (not condominiums)

There are two ways to complete a submission. The first way is to download our Adobe® .pdf version of the application. This will require Adobe Reader® or Acrobat®. You can update or download the software at Adobe.com®

The second way is to complete the web form and click on submit.

The first way, the download of our Adobe .pdf application, will allow you to complete the form online and "print" a copy for your records. You would then emai for fax the application with the required documentation.
